You will have already received/ completed a similar questionnaire for the hospital, however this is not provided to your anaesthetist prior to the day of surgery. To ensure Dr Heaney is appropriately prepared for your anaesthetic, please complete the anaesthetic pre-operative questionnaire.
The questionnaire will take 10-15 minutes to complete.

Before starting the questionnaire, please ensure you have on hand:
Your usual doctor’s contact details (GP & Specialists).
A list of your current medications.
Your weight and height.
A scanned JPG or PDF of any letters or results that you wish to send to Dr Heaney.

One full day prior to the procedure: Clear Fluid Diet only- no breakfast, lunch, dinner, or
snacks.
Do not eat any solid food, milk products.

For the full day prior to the procedure until midnight: You must only drink clear fluids or
consume items on the “Clear Fluid Diet”.
Clear Fluid Diet- The day prior to procedure

• Ice blocks on a stick – Lemonade / Orange / Lemon Flavour only
• Electrolyte drinks (Orange or lemon flavour only)

On the morning of the procedure (from midnight onwards) :
Drink up to 200 ml of fluids from the “Preoperative Clear fluid” list every hour until 2
hours before the procedure or as directed. If no advice given, then cease 2 hours before
admission to hospital/ clinic time.
No jelly, lollies, gums or snakes.
Pre-operative Clear Fluids (I.e., to drink on the day of the procedure. Clear = ‘see-through’)
